My stay at this facility was excellent the rehab was good. All my help was great. Social services Team help me with my time of discharge. The food could of been a bigger portion.

This place kind of sucks the way they do things don’t want to get into the particulars that you may have an open mind but by choice I wouldn’t come back some of the stuff we’re really nice and really tried I appreciated that. Also 2 CNA’s first Lucky very kind considerate always with a and bringing Joy looking out checking on you beautiful heart beautiful person thank you for your kindness and care. Also the night cna Lauren thank you for your care ,patience, attentiveness always checking if I needed anything thank you. If I could comment on the staff i would have no complaints except for a few that could have stepped up their game here and there but for the most part the staff was good treat you nice with respect except for one nurse which I had to put a complaint. My issue with the facility to me is there rehab program Bonita step it up. Because I couldn’t walk or get out of my bed they would say let’s try to stand okay I couldn’t but guess what there’s other things that I could do until I’m able to stand so when I wasn’t able to stand they just walked away and called it good that I did rehab I would have to tell them I wanted to do other exercises or there’s other things that I could do to get to the point where I can stand don’t just say you can’t stand and then walk away and then the sessions were only a few minutes there’s only one time did a guy work with me for 30 minutes which I really appreciated. The physical therapy people were lazy. After I was able to walk I wasn’t able to walk that long so I sat back down and I told him I wanted to do some exercises instead they brought me a paper and sit here and do homework it only took me 3 minutes to walk after that there should have been more time where I could have been doing some exercises but they threw papers on me sit here and do it on your own that wasn’t cool and that happened to me a few times

I made a bad review recently I should have went to social services but I didn’t no to I was new here but since my review everything is good my issues were addressed immediately and they went above and beyond. I have gotten my nails done hair fixed they suppose to set up dental and vision appts for me soon which are much needed do this is a 5 star place would recommend to everyone

This facility is an absolute joke! I can’t believe this facility has not been shut down, it is filthy and unsanitary. The staff is unprofessional and clueless when it comes to healthcare. Do not allow your loved one to stay at this facility.

This is three levels above the facility I was in 6-8 months ago. It’s cleaner! The staff is more friendly and on the ball! The food here is better! No complaints! Activities are fun and always full and everyone is smiling, happy and content.

I do not recommend this facility to anyone out there that wants the best for their family member. Not only do they not record instances that happen, like falls, injuries, etc. but they lie straight to your face about it when asked after seeing major bruises on our family members body. They do not give any type of quality care to the patients that they have there. Daily medications were also not given properly. Their staff is rude and does not care about the wellbeing of their patients. Several times I overheard them stating how irritating it was to have the patients just roaming the halls in their wheel chairs or just walking and trying to get out of the facility. Getting a nurse to respond in a timely manner was a joke. You could listen to them just talking and joking in the main hall vs answering call lights when patients needed help. After voicing my concern about a certain nurse, I was looked at like “ya we know they are not good but we are understaffed as it is.” I requested that nurse get off my family members rotation!! My family member went in just for rehab from a surgery and had to be taken out by an ambulance for going sepsis and into heart failure!!!!!!! THAT IS UNACCEPTABLE!!!! I begged for over a week to have tests run since our family member was not acting ok, not eating, and seemed off, and then once tests were finally done took several days to get results. THEY COULD HAVE DIED!!! Even after we went to the hospital we finally got the results from the tests that they did and the ER doctor said the tests they ran were a joke and should never have taken 3 days to get the results, when the tests the ER ran took 30 minutes. The ER stated if I wouldn’t have gotten him to the ER, he might not have had a good chance at recovering from the severe infection that facility ignored!!!! I could go on about how unprofessional this facility is and hope that no one has to put a loved one in there ever, I would fear for their health and safety. Hopefully my family member makes a full recovery but the damage this place caused we aren’t sure. This facility needs to be inspected by the state or someone, the staff needs to care about their patients or find another job!! DO NOT TAKE ANY LOVED ONE HERE!!!! If i could give negative stars i would!! UPDATE: DUE TO THE NEGLIGENCE OF THIS FACILITY AND STAFF MY FATHER PASSED AWAY FROM THE DAMAGE THE SEPSIS HAD CAUSED ONCE TAKEN TO THE ER!!!!! DO NOT TAKE ANYONE HERE IF YOU WANT THEM TO SURVIVE.. THIS PLACE NEEDS TO BE CHECKED BY THE STATE AND SHUT DOWN!!

Please do not send a loved one here if you can help it. If a family member is here, be prepared to advocate every SINGLE step of the way. I work in healthcare and I’ve worked in hospitals and long term care facilities etc, and to say this experience was completely horrid says a lot. There were a few people who worked hard for my mom, I will not underplay those few caregivers because I am thankful for those that cared, but ultimately the entire place is so poorly run and does not lead to success for likely most patients who come here. My family member had a complicated stroke, which just requires individualized time and attention to therapies, movement, exercises, etc. They’re too busy and understaffed to give that, or really even the bare minimum. My mom was extremely motivated but was not given much (or really anytime) for regular exercises or movement outside of her 30-60min therapy a few days a week. My mom sat in bed for most of the time, and was forced to eliminate herself sitting in bed for MUCH longer than needed. During the first few weeks of her healing and determining her capabilities, sure… but after that? No. Sheer negligence. She was completely capable of utilizing a bedpan or a commode, but no one took to time to evaluate that until I mentioned it several weeks in and got her up myself. Can we try and preserve someone’s dignity and actually try to get someone back to their independent self? In her time at this facility she fell, wore the same clothes for multiple days straight, and got showered MAYBE twice in the month/month and a half she was there. Talk about neglect? The facility did not help whatsoever with arrangements going home, they didn’t make sure we had supplies, a wheelchair, a bed, NOTHING. Myself and my family were responsible for organizing, calling, advocating, setting up her health insurance and everything. What exactly is a social worker for then? 🙁 Additionally, the staff sent her home with medications that were not correctly labeled or prescribed for the correct route. Come on people… totally unacceptable and asking for a medication error. Anyone else not medically inclined would be completely screwed and would result in harm.

I was at this facility from February 24 to April 24. I had a wonderful experience. The nurses and CNA’s were extremely helpful and supportive. They were re painting and fixing up the rooms and i had a lovely room. Sometimes, if they were short handed, they let the patients know, and worked diligently to serve each patient as quickly as possible. The CNA’s and nurses were all professional and extremely friendly. If needed , i would come back again. Thank you to all the staff for the excellent care that I received.

So far I am not happy with the care they are providing to my mother . I gave the nurse prescription medication my mom is supposed to be taking . They are ignoring our requests to give her the medication.. this is for her mental health it has nothing to do with why my mom is in that place . Give her the medicine and stop wasting time . Also, it’s really hot in there and people are cramped 3 per room I don’t know if this is common but the other places I’ve been to when providing manicure services , those other rehabilitation places provide people with their own room ….it is super hot in there and it doesn’t smell very good. I know the work isn’t easy so I will give the staff credit for that. but so far I am not satisfied with them for taking my mom’s medication from me, but not actually providing it to her. Her mental health isn’t well and they are not helping the situation .I would like to have her transferred out of this place .

Been calling all night and nobody answers the phone . It says on the website that they’re open 24 hours but that is not true because they don’t answer the phone!! My sister is in pain and her call light has been on for 57 mins.. What’s going on with this facility? Why the lack of patient care? If this place is understaffed the D.O.N needs to do something about it!! BEFORE YOU DECIDE TO PUT YOUR LOVED ONE IN THIS FACILITY I SUGGEST YOU READ THE REVIEWS..

I missed home but I felt like the staff cared, there are 3 CNA that are amazing: Teresa, Lauren and Natalie the ladies are earth angels they made my stay comfortable, the nurse are awesome as well, the physical therapy crew are wonderful there pushed when needed, I will miss the staff

I was referred to this facility after a bad fall at home. Thankfully I didn’t break anything but I needed rehabilitation after being immobile for at least 10 years due to multiple sclerosis and other chronic illnesses. The nurses and physical therapists were fantastic, personable and treated me like a member of their own families with the exception of one nurse who was cold and seemed like she was put out that she had to help me. All the other nurses were absolutely wonderful and caring and were happy to care-give and had a natural gift of taking pride in their profession even when it was emptying a bedpan or other unpleasant jobs related to being a CNA. 1 day in particular I waited for 3 hours for the nurse to help me after pushing the call button several times! Most days though, I was tended to almost immediately. I would like to give a special shoutout to (MARIA, ALYSSA and TASHA and a few others) due to being extremely wonderful CNA’s. SUSAN gave great hospitality to the patients overall.There were a few others but their names elude me now. I was put in the old section of the building which was outdated and wasn’t very appealing but all things considered, I’d recommend this place due to the wonderful nurses that work there. 4 stars ⭐️ ⭐️⭐️⭐️!

My mother stayed at this place for less than 3 days . She was neglected by the staff and was not given her medication properly. The staff did not communicate with each other and never gave her proper care or therapy. We took her out since she had a horrible rash, her blood pressure was so high and had severe headaches and was nauseous. When the staff was confronted with this they did not even apologize and did not admit to their lack of humanity and professionalism. Please don’t take your family to this place let them go out of business as they truly do nothing to help an elderly and defenseless person. I will continue to post until they get audited by the state of California.

Thank you for a wonderful experience, all the bad reviews I see here can’t be real. This place is the only one with caring cnas like Lucky and Teresa and they have nurses with actual knowledge that go above and beyond. I’ve been to other nursing and rehab facilities like this and they don’t even do the minimum for their patients. I’d come back here every time, if I had to do this again. Thank you San Joaquin

One of the best nursing facilities I have been to. A lot of great friendly staff to talk to and lots of activities to do between friendly CNA therapy an activities staff I was actually sad to leave because I made so many friends an staff treated me as a person activities had things for us to to in a group an if I wanted to be in my room the accommodated that as well never a dull moment I love how everyone has a good sense of humor as well

My step dad is in this place and they put him in the far back corner. I showed up to visit him @ 1pm and he still had not been given his am meal or his noon meal. When he presses his call button it takes the staff up to 1 1/2hrs before they tend to his needs. There theripy is a joke. Rooms are dingy and under stocked with medical supplies. This place does the bear minimum so the can keep you longer for the insurance money. My step dad is getting bed sores. They leave you in soiled diapers. The food sucks. They lose your personal clothes in Landry. If you love your family don’t let them go to that place.

My mother was in this facility. Got out of the facility past two nursing stations. I got a call at work that she was missing. I had to leave work to go find her but yet the facility neglected to report to the state about her getting out.

My grandpa is here and we have had and amazing experience with lakhwinder Kaur, when she is here my grandpa is always cleaned and shaved.We never have to push the call button when she is here because she’s on top of checking on him.She is so kind and patient.
